A pickup measure is an incomplete measure at the beginning of a piece of music. It is also called an anacrusis. You need to create a pickup measure for music that does not begin on the first beat of a measure. Example: The tune "Tristeza" begins with a pickup.

Regarding your actual problem:
You can't have an ordinary note in the first half of a measure, while the second half of the measure is a mirror.
So the solution is notating the music in measures of 2/4 while displaying 4/4, using invisible barlines every second measure:
1. Time Signature Tool. Click the first measure. The Time Signature dialog box appears.
2. Click the "More Choices" button. Turn on "Use a Different Time Signature for Display".
Set the "Time Signature for Display" to 4/4. Set the actual Time Signature to 2/4. Click OK.
3. Measure Tool. Double-click the first measure. The Measure Attributes dialog box appears.
Set the barline to "Invisible". Click OK.
Select all the measures except the first measure (measures 2 to 32). Double-click one of the selected measures. Set the barline to "Invisible". In the bottom of the dialog box, set the measure attributes to appear every 2 measures. Click OK.

For creating all these mirrors I recommend that you use the Mass Mover Tool.
In the Mass Mover menu, select "Mirror".
Click measure 2 (which on the screen seems to be the second half of the first measure) to select it. Now hold down the secret modifier key combination (Windows: Ctrl-Shift ; Mac: Option-Shift), and click measure 4 (which on the screen seems to be the second half of the second measure), measure 6, measure 8, and so on.
