Name that Song

Here's a bit of a melody line from a piece of music that I'm trying to find. I'm not sure who wrote the music but I'm under the impression it's from a composer years ago, classical maybe, and I'd like to know the name of the piece or more info on it. Maybe someone on here will recognize what I put down in this "Sample" file... nothing sophisticated, just some of what I remember. Thanks.

If I recall correctly, the waltz’ original key signature is D Major.
