And what is your problem?
Do you need chord symbols - as an analysis of the harmony?
Or is your music theory problem something else?

The triplets in the lower staff seem wrong.

How so? It looks right to me, in 12/8. But, what do I know?

The triplets in the left hand don't make any sense, but I hear the piece in 3/4, like this, with sixteenth-note broken chords in the left hand.

What you have is technically correct, but a bit awkward to read. If this is what I wanted I might perhaps try something like this, depending on tempo and other factors.

Depending on the context (= the previous measures that we do not know) the first measure could also be notated in sharps - so that both measures begin the upper staff with a B♮:
