In this piece, we sing through the entire piece twice, verse and chorus. We repeat it again, but at the end, we shorten the last measure of the chorus by 1 beat, add in the pickup notes, and repeat the chorus again, ending with the full duration.
1. Did I lay it out correctly, or is the 1, 2, 4 not done?
2. Should the chords go under the bracket? If so, how do I set it up so that the repeat numbers aren't covered?
3. Would it be better to do it as a DS al CODA, so: 1,2 / 3 / CODA?

It's possibly a little confusing, and would take some explanation followed by people scribbling in their music, but the only alternative I can think of would be to have the third ending continue on and write out the verse again (and thus eliminate the fourth ending), which I think would stand on its own and need no 'splaining.

To me they are perfectly understandable.
As another solution you could add the fourth ending after the third ending - it only adds two measures to the document.
But I would keep the chord symbols under the repeats.
In Other Words: the repeats above the chord symbols.
That is: move the repeats up, to make room for the chord symbols.
This can be done “globally” (in Document Options - Repeats).

I also find it discomforting. If I had plenty of time to adapt to the non-linear flow in rehearsal, I suppose the existing layout is OK. But if I had to perform the piece without such luxury, my performance would likely suffer a bit, having to divide my concentration between reading, singing and guarding against missing the directive.
