Notes leave out accidentals, but play back properly

I get this every once in awhile: let's say it's key of D, and I enter a D#....the note will play back as D#, but in the score, it appears as a D natural. Strangely, if I start on that D (appearing as D on the page, but honoring my notation by sounding as D#) and move the note up by half steps, every note is moved up a half step.....so what would be the notes of a D Major scale, will be a D# (Eb) major scale. Tried clearing midi data, not sure what else to try. Any ideas?

Not using MIDI keyboard. Here's the bar in question, which is Alto Sax bar 2, on the D natural (which is playing back as D#, AS INTENDED, but notation shows it as D natural.) I look forward to this being some simple thing I'm overlooking.....

In the transposed score, it is D# which is covered by the key signature so no accidental is necessary.

In the transposed score the note is B♯.
But in the attached Finale document the score is in Concert Pitch = not a transposed score; the option {Display in Concert Pitch} is selected, and the note is D♯.
The error is that the D♯ is tied to the previous note (= E).
But a tie should be used only between two notes of the same pitch (= from an E to an E, or from a D♯ to a D♯).
In the Simple Entry Tool, select the D♯, and hit Shift-T, to remove the tie end.
Also move back onto the E, and hit the letter T key, to remove the forward tie.
You can connect the E and the D♯ with a slur.

Thank you Peter! That was it....I understand what the tie is for, but sometimes I'll have a tied note and then go back and revoice something, and in this case I did that and neglected to remove the tie, thus the issue. I went back and fixed it and all is well. Much appreciated.

We are both right.

Good point, I didn't specify Alto one or two. But I did say bar 2 with the d..... I think that was pretty specific as stated.... However, the fact that there was any confusion at all is my fault--and I will make sure in the future to avoid any such issue.
