Wondering how to create a slur between the 1 and the 4 under the
lower D#. This is to indicate the fingering change between thumb and
4th finger. See attached, thanks.

You can use one of the elision symbols to be found in the Engraver Font. They look like little slurs. Then you position it by hand over or under the numbers. Or you can use an actual beat-attached slur. This becomes necessary when the numbers get further apart.
